Telemedicine has become a valuable tool in healthcare, offering increased accessibility and convenience for patients and health professionals. However, not all reasons for consultation are suited to this form of care. Here is an analysis of the most common reasons for consultation and their relevance to telemedicine.

Reasons for Consultation Suited to Telemedicine

  1. Follow-up Consultations Requested by the Doctor

• Patients with chronic diseases such as diabetes, hypertension, or asthma can benefit from follow-up consultations via telemedicine. This allows doctors to monitor health parameters, adjust treatments, and provide advice without requiring a physical visit for every consultation.

  1. Minor Health Problems

• Minor ailments such as colds, simple urinary tract infections, sinusitis, conjunctivitis, or skin rashes can often be diagnosed and treated via a virtual consultation. Doctors can prescribe medications or advise on home treatments.

  1. Mental Health Issues

• Consultations for disorders such as anxiety, depression, or stress without physical symptoms (chest pain, abdominal pain, etc.) can be effectively conducted online with mental health professionals.

  1. Dermatological Problems

• Many skin problems can be evaluated via telemedicine, often using photos sent in advance. Photos generally offer much higher resolution than webcam cameras.

  1. Eye Redness

• Minor ocular problems, such as eye redness without pain and without vision impairment, can often be evaluated remotely.

  1. Sexual Health

• Consultations for the screening and treatment of asymptomatic sexually transmitted infections (STIs) and hormonal contraception are well-suited to telemedicine.

• Consultations to obtain general medical advice, information on disease prevention, or instructions on managing certain health conditions are well-suited to telemedicine.

Reasons for Consultation Less Suited to Telemedicine

Health problems that cannot be treated without an in-person physical exam are not suitable for virtual care. For example:

  1. Medical Emergencies

• Situations requiring immediate medical attention, such as chest pain, difficulty breathing, heavy bleeding, or stroke symptoms, must be treated in person. Telemedicine is not suited for managing these emergencies.

  1. Chest Pain

• Chest pain often requires an in-person evaluation to rule out serious conditions such as myocardial infarction.

  1. Shortness of Breath

• Shortness of breath can be a sign of serious conditions such as heart or lung disease, requiring a physical evaluation.

  1. Vision or Hearing Loss

• Sudden or unexplained loss of vision or hearing must be evaluated in person to identify potentially serious causes.

  1. Sudden Weakness or Lethargy

• These symptoms may indicate a stroke or another serious condition requiring immediate evaluation.

  1. Ear Pain

• Ear pain requires a physical exam for an accurate diagnosis.

  1. Persistent Cough

• A persistent cough requires a physical exam and potentially additional tests for an accurate diagnosis.

  1. Abdominal or Digestive Problems

• Abdominal or gastrointestinal symptoms often require a physical exam to determine the exact cause.

• Musculoskeletal injuries often require a physical exam to assess the severity of the injury.

  1. Complex Symptoms

• Complex symptoms such as dizziness, loss of consciousness, or undiagnosed chronic pain may require physical tests or investigations that cannot be performed via telemedicine.

  1. Major Consultations in General (except Mental Health)

• Major consultations requiring a thorough and complete evaluation, with the exception of mental health, must generally be done in person to ensure adequate management.

  1. Periodic Health Check-up

Health check-ups often require a physical evaluation for a complete analysis of the patient's health status.

  1. Referrals to a Specialist Physician

• Before referring a patient to a specialist, a complete clinical evaluation must be performed by the treating physician, which may require an in-person consultation.

  1. Prescription Renewals

• Prescription renewals without a medical re-evaluation within the last year are not suited for telemedicine, as an update on the patient's health status is essential. We can only provide temporary refills for a few months until an in-person consultation occurs, but please note that the pharmacist may also be able to do this.

Conclusion

However, it is essential to understand its limits and know when an in-person consultation is necessary to ensure appropriate care.
